Oak Street creates a bright spot in low-income neighborhoods in several cities, building its health clinics with a community center, offering classes and educating the community on the value primary care. Oak street treatment teams taking time with each member, building in constant outreach and wraparound services like behavioral health, pharmacy and transportation to improve member health. And results are impressive: clinics see over 40% reduction in emergency department visits by its members.
